Land and labour costs in the Klang Valley, Penang, and Johor Bahru are higher than in secondary cities. Facilities in Petaling Jaya or Mont Kiara command a premium over otherwise similar homes in Rawang or Seremban.
3. Room type
Dormitory (6–8 beds) is cheapest. 4-bed wards, twin rooms, and private suites each step up in price. For many families, a 4-bed ward is a reasonable balance — private enough, without the single-room premium.
4. Staff ratio and qualifications
Facilities with a higher ratio of trained nurses to residents — and 24-hour nursing cover — have higher fixed costs. This is a legitimate reason for a higher monthly fee.
5. Brand and environment
Newer facilities in purpose-built buildings with gardens, therapy pools, or activity wings cost more to operate and reflect that in fees. Whether the premium is worth it depends on how much the resident will use these facilities.
